29 CFR 1910.269(l)(1): Unqualified employees were allowed to work on or with exposed energized lines or parts of equipment.  On or about October 10, 2014 on the worksite on Gateway Road, Gainesville, Georgia, an employee entered the energized side of a 14.4 three phase air switch to verify the cable phasing prior to cable termination. The employee was working in the live side of the air switch when he connected one end of a handset to the ground and the other end to the bus bar on one of the phases causing an arc flash, exposing the employee to second and third degree burns.
