29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  a) Jobsite - On or about October 29, 2014, employees working inside a 13-foot, 9-inch deep excavation were not safeguarded from head injuries such as but not limited to soil from the spoil pile, dirt from the excavator bucket that was being used inside the excavation while employees were working in it, and being hit in the head by the excavator bucket itself.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a) Jobsite - On or about October 29, 2014 employees were not wearing impact resistant eye wear while using  a chop saw to cut concrete piping thus exposing employees to eye injuries from being struck by flying debris such as pieces of concrete.

29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:  a) South East Corner of Excavation - On or about October 29, 2014 employees accessed an excavation up to about 13 feet, 9 inches in depth to perform work without a safe means of egress including but not limited to a ladder or dirt ramp, exposing employees to fall and/or engulfment hazards.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or 1926.652(c):  a) South East Corner of Excavation - On or about October 29, 2014, employees were allowed to access and perform work for installation of a manhole in an excavation with Type B and Type C soil and up to about 13 feet, 9 inches deep without adequate cave-in protection, exposing employees to crushed-by and engulfment hazards.
