29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: a) Jobsite - On or about October 29, 2014, employees working inside a 13-foot, 9-inch deep excavation were not safeguarded from head injuries such as but not limited to soil from the spoil pile, dirt from the excavator bucket that was being used inside the excavation while employees were working in it, and being hit in the head by the excavator bucket itself.
