29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   JOB SITE: 9948 Meadow Avenue, Saint Louis, MO 63125, on or about 10/20/2014, at this work site, workers were observed in rear of the residential structure, on top of a 4:12 pitch roof, actively tearing down the old roof, approximately 10 feet 5 inches eave to ground level and were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measures.  The workers were exposed to a fall hazard.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed  JOB SITE: 9948 Meadow Avenue, Saint Louis, MO 63125,on or about 10/30/2014, at this work site, workers were observed using an Aluminum Extension Ladder in front of the residential structure, approximately 10 feet 5 inches eave to ground level, and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.
