29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a) Swiss Construction on or about October 29, 2014, at the address of 4930 Transportation Dr. Sheffield, OH: The employer did not ensure an employee was protected from head injuries while holding/controlling a tag line attached to a trusses being hoisted via forklift. The employee was exposed to potential impact injuries associated with falling framing material.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a) Swiss Construction on or about October 29, 2014, at the address of 4930 Transportation Dr. Sheffield, OH: The employer failed to ensure employees were protected from falls while performing wood framing work on a commercial structure.  During this work, employees were exposed to falls up to 22 ft. to the ground below.
