29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by a guardrail system, safety net system, or personal fall arrest system, or an alternative fall protection measure that meets a provision described in paragraph (b) of 29 CFR 1926.501.  On or about September 29, 2014, the employer did not ensure employees working from a roof were in use of fall protection exposing the employees to a fall hazard of more than 6 feet to the ground below.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.     On or about October 29, 2014, the employer did not ensure the side rails of an extension ladder being used by employees to gain access to the roof of a building extended at least 3 feet above the landing exposing employees to a fall hazard.
