On or about October 31, 2014, on the east side of the construction site, employees cutting framing material used a Makita skill saw that had the power cord outer sheathing cut/worn exposing employees to electrical shock hazards.

On or about October 31, 2014, on the east side of the construction site, employees cutting framing material used a Makita skill saw with the blade guard wedged open in the full up position preventing the guard to retract and cover the rotating blade exposing the employees to struck-by and amputation hazards.

On or about October 22, 2014, on the east side of the construction site, the employer did not provide a ladder where employees could safely access and egress the building upper landings exposing the employees to a potential fall of more than 11 feet.
