29 CFR 1926.502(d)(16)(v): Personal fall arrest systems, when stopping a fall, did not have sufficient strength to withstand twice the potential impact energy of an employee free falling a distance of 6 feet (1.8 m) or the free fall distance permitted: a) Garage roof of residence at 1106 Cooperwood Drive, Marietta, Georgia. An employee, installing metal weatherproofing flashing from a steep, 10 in 12 pitched roof was exposed to fall hazards of 10-feet 2-inches and 18-feet 1-inch. The employee was wearing a personal fall arrest system. However, the anchor point of the system, a five eighths inch synthetic rope nailed into the roof, would not be able to withstand twice the impact load if the employee fell.

29 CFR 1926.1053(b)(1): The side rails of portable ladders used for access did not extend at least 3 feet above the upper landing surface, or was secured at its top to a rigid support that will not deflect and along with a grasping devise to assist employees mounting and dismounting the ladder. a) Garage roof of residence at 1106 Cooperwood Drive, Marietta, Georgia. An employee was exposed to a fall hazard of 10-feet 2-inches while mounting and dismounting an extension ladder used to access an upper landing. The side rails of the ladder did not extend at least three feet above the upper landing and the ladder was not secured to prevent movement or displacement.
