29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary: -Employees were working in the bottom of an open excavation measured to be 15'-6" in depth while being exposed to materials and equipment falling into the excavation on top of them, posing a crushing/struck-by hazard.

29 CFR 1926.652(b)(2): Maximum allowable slopes, and allowable configurations for sloping and benching systems, was not determined in accordance with the conditions and requirements set forth in Appendices A and B to this subpart: -The combination of trench shield and sloping was not constructed in accordance to appendix B in that the slope of the sidewall was higher than the trench shield exposing employees to possible cave in hazards from the exposed sidewall and struck by/crushing hazards from material being able to roll/fall into the excavation.
