On or about October 21, 2014, employees were installing the metal edge of an awning without fall protection, exposing the employees to a fall hazard of more than 6 feet to the ground below.

29 CFR 1926.1053(b)(1):  A portable ladder used for access to an upper landing surface did not have the ladder side rails extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access or when such and extension is not possible because of the ladders length, then the ladder will be secured at its top to a rigid support that will not deflect, and a grasping device such as a grabrail, will be provided to assist employees in mounting and dismounting the ladder.  On or about October 21, 2014, at a new structure construction site, employees were allowed to use a ladder that was not extended at least 3 feet above the landing or secured to the structure to ensure safe access/egress, exposing employees to a fall hazard.
