29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. The employer did not ensure fall protection was implemented for employees performing construction work on a roof. Employees were exposed to fall hazards associated with an approximately forty (40) foot fall.

29 CFR 1926.502(e)(8)(ii): A snaphook not of the locking type or designed for the following connection was engaged to (i) directly to webbing, rope or wire rope; (ii) to each other (iii) to a Dee-ring to which another snaphook or other connector is attached;(iv) to a horizontal lifeline; or (v) to any object which is incompatibly shaped or dimensioned in relation to the snaphook such that unintentional disengagement could occur by the connected object being able to depress the snaphook keeper and release itself. Employees were utilizing lanyards whose snaphooks were connected directly to the snaphook of another lanyard while performing roofing operations at a height of approximately forty (40) feet above ground.
