29 CFR 1926.307(e)(1)(i): Where both runs of a horizontal belt were 42 inches (106.68 cm) or less from the ground, the belt was not fully enclosed:   a) Front of residence located at Lot 26 of the Highland falls subdivision in Hiram, Georgia. The employee operating the shingle lift was exposed to an unguarded belt and pulley drive system. The belt and pulley drive of the shingle lift, located at the base of the lift, was not enclosed.

29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:   a) Roof of residence located at Lot 26 Highland Falls subdivision Hiram, Georgia. An employee, who had attached the lanyard of a personal fall arrest system improperly, was exposed to a fall hazard of 18-feet-4-inches. The employee had attached the lanyard front of him at waist level not as prescribed, at his back at shoulder level or head level.
