(a) 22 Hendrick Street, Easthampton, MA: On or about November 4, 2014, employees were observed working around live service entrance conductors rated at less than 600 volts and were inside of minimum clearance limits of 10 ft.. (1) Pump jack scaffold, north wall employees working within 10 ft. of live service entrance.

(a) 22 Hendricks Street, Easthampton, MA: On or about November 4, 2014, employees were observed working on pump scaffolds which were not secured at both the top and bottom to prevent slippage. (1) South wall, pump jack scaffold. (2) North wall, pump jack scaffold.
