On or about 11/4/14 located at 5230 Rustic Ct., Cumming, GA: Employees installing plywood on the roof while working at the edge, the employer did not ensure that fall protection was used by the employees while exposed to fall hazards of approximately up to 35 feet and the pitch was a 6/12 to 12/12.
