29 CFR 1910.1048(d)(1)(i): Employees of a workplace covered by this standard were not monitored to determine their exposure to formaldehyde:  (a) In the salon and spa:  On and before November 4, 2014, the employer did not monitor employees potentially exposed to formaldehyde to determine if the employee exposure was below the action level (0.5 ppm, 8-hr TWA), the 8-hour time weighted average (0.75 ppm) or the 15-minute short term exposure level (2 ppm).   NOTE: Abatement certification IS required for this item.

29 CFR 1910.1200(e)(1): Employer had not developed or implemented a written hazard communication program included the requirements outlined in 29 CFR 1910.1200(e)(1)(i) and (e)(1)(ii):  (a)  In the salon and spa: On November 4, 2014 the employer did not have a written hazard communication program which included label and warning requirements, safety data sheets, and employee training for chemicals contained in common products used in beauty salons such as formaldehyde, acrylates, acetones, alcohol, hair dyes, nail polishes and other chemicals which are known or suspected to be carcinogens, teratogens, respiratory irritants, or central nervous system depressants.   NOTE: Abatement certification IS required for this item.
