On or about Nov. 04, 2014, on the southeast side (2nd floor) of the commercial structure, an employee sweeping and cleaning up debris from a balcony was not protected from a 8 foot 6 inch fall hazard by the use of a personal fall protection system.  b.  On or about Nov. 04, 2014, on the southwest side (5th floor) of the commercial structure, an employee repairing the concrete slab to the balcony was not protected from a 33 foot fall hazard by the use of a personal fall protection system.  c.  On or about Nov. 04, 2014, on the southwest side (6th floor) of the commercial structure, an employee repairing the concrete slab to the balcony was not protected from a 41 foot 1 inch fall hazard by the use of a personal fall protection system.  d.  On or about Nov. 04, 2014, on the southwest side (8th floor) of the commercial structure, an employee repairing the concrete slab to the balcony was not protected from a 56 foot 3 inch fall hazard by the use of a personal fall protection system.
