29 CFR 1926.25(a): During the course of construction, debris and scrap lumber with protruding nails was not kept cleared from passageways, work areas and in and around building:     a) 916 S. 6th Street, Philadelphia, PA - The company did not ensure during the course of construction that debris, materials and scrap lumber with protruding nails were kept clear from passageways, work areas and from around the building. This hazard exposed the employees to puncture and trip/fall hazards. This condition was observed on or about November 5, 2014.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms:    916 S 6th Street, Philadelphia, PA - The employees were working from a pole scaffold approximately 15 feet above the ground without having guardrails installed along all open sides and ends. The employer did not ensure that the employees were completely protected from falling off of the pole scaffold by installing endrails on the work platform thus exposing the employees to a fall hazard. This condition was observed on or about November 5, 2014.

29 CFR 1926.451(g)(4)(iv): Rails were not installed midway between the top edge of the guardrail system and the platform surface:    a) 916 S 6th Street, Philadelphia, PA - The employees were working from a pole scaffold approximately 15 feet above the ground without having a completed guardrail systems, mid-rails were missing on the guardrail system. The employer did not ensure that mid-rails on the work platform were installed thus exposing the employees to a fall hazard. This condition was observed on or about November 5, 2014.
