Broadway, Wichita, KS 67216, Chrisman's Truck & Auto Salvage employees operating equipment were exposed to crushing and/or struck-by hazards in that the training program did not include formal instructions related to the safe operation of the 621-C Case loader.    One feasible and acceptable methods to abate this hazard is to train operators on the use and limitations of front end loaders.

29 CFR 1910.244(a)(2)(i): In the absence of a firm foundation, the base of the jack was not blocked. Where there was a possibility of slippage of the cap, a block was not placed in between the cap and the load:   On or about November 7, 2014 at a worksite located at 3852 S. Broadway, Wichita, KS 67216, an employee was exposed to struck-by/crushing hazards.  In order to retrieve a car part, the front passenger side of a 1997 Pontiac 3800 V6 Grand Prix SE was raised up using two screw/scissor jacks on the gravel surface of the used car salvage yard. The base of the jacks were not blocked, nor was a block placed in between the cap and the load to prevent the jacks from slipping.   29 CFR 1903.19(d)(1) requires certification and documentation that the abatement of the above violation is completed.
