29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:   (a) On or about October 29, 2014, Carlos Villanueva did not ensure that eye and face protection were used by employees when machines or operations presented potential eye or face injury. Employees of Carlos Villanueva were observed nailing plywood on a residential roof, using a pneumatic gun, without any eye or face protection. 29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:   (a) On or about October 29, 2014, Carlos Villanueva did not ensure that its employees were protected from falls while working at heights greater than 6 feet. Employees of Carlos Villanueva were exposed to fall hazards in excess of 11 feet while engaged in roofing activities on the roof of a residential building with no fall protection in place. 29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access:   (a) On or about October 29, 2014, Carlos Villanueva did not ensure that portable ladders, used to access upper landing surfaces, extend at least 3 feet above the upper landing surface to which the ladders were used to gain access. Workers were observed using a ladder to access the roof of a residential building. Ladder did not extend at least 3 ft above the landing platform and was not tied to the roof edge.
