29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:    On or about 11/07/2014 employees at the Albany Turnpike siding project were exposed to approximately 18' falls. An employee was working from an aerial lift and using personal fall protection that was not rigged to prevent contact with the lower level.    Shepardville Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.502(d)(16)(iii), which was contained in OSHA inspection number 949030, citation number 01, item number 01 and was affirmed as a final order on 12/20/2013, with respect to a workplace located at 2875 Main Street, Glastonbury, CT 06033.

29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee has been trained, as necessary, by a competent person in the use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used.    On or about 11/07/2014 employees at the Albany Turnpike siding project were exposed to approximately 18' falls due to an improper fall protection lanyard.  The employer did not train on the use, limitation and restrictions of the fall protection lanyard in use.    Shepardville Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.503(a)(2)(iii), which was contained in OSHA inspection number 949030, citation number 01, item number 02 and was affirmed as a final order on 12/20/2013, with respect to a workplace located at 2875 Main Street, Glastonbury, CT 06033.
