29 CFR 1926.304(f): All woodworking tools and machinery did not meet other applicable requirements of American National Standards Institute, 01.1-1961, Safety Code for Woodworking Machinery, in that:   a) Porch - On or about 11/07/2014, a spreader to protect carpenters from wood  trim and other materials fed into the saw binding and being ejected,  was not provided as required by the American National Standards Institute, 01.-1961, Safety Code for Woodworking Machinery, Section 4.1.2(b).          b) Porch - On or about 11/07/2014, an anti-kickback device or dogs to protect carpenters from wood  trim and other materials fed into the saw binding and being ejected was not provided, as required by the American National Standards Institute, 01.-1961, Safety Code for Woodworking Machinery, Section 4.1.2(e).    c) Porch - On or about 11/07/2014, a restart switch that would prevent the saw from unexpectedly restarting after a temporary interruption of power was not provided, as required by the American National Standards Institute, 01.-1961, Safety Code for Woodworking Machinery, Section 3.1.3(c).            Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.304(i)(1): Each circular had-fed ripsaw was not guarded by a hood which completely enclosed that portion of the saw above the table and that portion of the saw above the material being cut:   a)  Porch - On or about 11/07/2014, a Makita 10 in. portable table saw (serial no. 89224-1414) was not provided with a hood or top guard to prevent the carpenter from accidentally coming into contact with the saw's rotating blade.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)       Porch Roof  A worker was exposed to a 12 foot 6inch  fall from a low-sloped roof while installing exterior siding and was not provided with fall protection.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:  a)   Porch Roof - A worker was exposed to a 12 foot 6 inch fall from the top of an aluminum extension ladder which projected only 2 feet above the ladder landing, was not secured in place and was not provide with a grabrail.
