29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: a) 5th Street, NW, Exposure - On or about 11/07/2014, two mason tenders working at or around the base of a fabricated frame scaffold were exposed to a struck-by hazard from unrestrained loose brick and hand tool and were not provided with head protection.

29 CFR 1926.451(c)(1)(ii): 29 CFR 1926.451(c)(1)(ii): Supported scaffolds with a height to base width ratio of more than four to one (4:1) were not restrained from tipping by installing guys, ties, and braces at each end of the scaffold and/or at horizontal intervals not to exceed 30 feet (9.1 m): a) 5th Street, NW, Exposure On or about 11/07/2014, masons laying brick from a two fabricated frame scaffold systems were exposed to a potential scaffold collapse for the scaffolds, which were both 24 feet tall and five feet wide, were not otherwise restrained of attached or secured to the face of the building by guying or some similar method of providing stability. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
