29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:      (a) On or about October 30, 2014, and at times prior, Sierra Glass, Inc. did not ensure that its employees were protected from fall hazards while installing windows on a commercial building. Sierra Glass, Inc. did not ensure that its employees wore restraining devices such as body belts or harnesses with lanyards attached to the boom or basket of a Genie S-60X aerial lift, exposing employees to fall hazards in excess of 13 feet.         Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
