29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:    (a) On or about October 30, 2014, the employer did not ensure that its employees used protective equipment whenever hazards capable of causing injury and impairment were encountered. Employees of Swingle Lawn, Tree & Landscape Care were exposed to fall hazards in excess of 15 feet while engaged in lighting installation activities on the roof of a commercial building, without fall protection equipment properly tied off to a roof anchor.        Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
