29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:    a) Jobsite, Roosevelt Lane, Nemacolin, PA:  On or about November 10, 2014, 3 employees were installing shingles on a commercial roof and were not protected from falling.  The fall hazard was approximately 18-27 feet to the ground.

29 CFR 1926.503(c): The employer did not retrain affected employees  who already had been trained but demonstrated inadequate  understanding and skill required by paragraph (a) of this section:    a) Jobsite, Roosevelt Lane, Nemacolin, PA:  On or about November 10, 2014, three employees were working on a commercial roof without fall protection.  They had been trained previously, but did not demonstrate the knowledge and/or ability to recognize the hazards of falling and the procedures to be followed in order to minimize the hazards.
