29 CFR 1926.451(b)(1):  Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):       (a) On or about November 7th, 2014, at a job site located at 5141 Main Street in Kansas City, MO, an employee was exposed to a fall hazard while working from a rolling scaffold that was not fully decked as required by standard.  The working "platform" consisted of a single prefabricated scaffold plank ("pick board") that was only 18 inches in width.       Abatement certification is required for this Item.  Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.     NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.451(g)(1)(vii):  Each employee on a scaffold not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above the lower level, was not protected from falls by the use of a personal fall arrest system or guardrail system meeting the requirements of paragraph (g)(4) of this section:      (a) On or about November 7th, 2014, at a job site located at 5141 Main Street in Kansas City, MO, an employee was observed to be working from a rolling scaffold, over 10 feet above the lower level, who was not protected by a guardrail system meeting the applicable standards.      Abatement certification is required for this Item.  Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.    NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

(a) On or about November 7th, 2014, at a job site located at 5141 Main Street in Kansas City, MO, two employees were observed to be working from an elevated scissors lift without the safety chains secured on the open (access) end of the platform.   Abatement certification is required for this Item.  Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.  NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.452(w)(2): Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold was used in a stationary manner:      (a) On or about November 7th, 2014, at a job site located at 5141 Main Street in Kansas City, MO, an employee was working from a rolling tubular welded-frame scaffold.  The caster wheels were not locked or otherwise restrained from movement.  At least two of the wheel locks were not functional in that they did not contact the caster wheels even in their "locked" position.      Abatement certification is required for this Item.  Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.    NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
