29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolding was not fully planked or decked:  a.  Stone Haven Subdivision, 1675 Glen Haven Way, Lot #78C,  Lawrenceville, GA (Jobsite) - Employees were exposed to fall hazard.  Employees were working from a tubular welded frame scaffold, removing all the work materials from the scaffold before starting the process of dismantling.  The working levels which the employees were working from was not fully planked, creating a fall hazard of approximately 18-24 feet above ground level, on or about 11/13/14.

29 CFR 1926.451(e)(1): Where scaffold platforms were more than 2 feet above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers, stairway-type ladders, ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used for access:  a.  Stone Haven Subdivision, 1675 Glen Haven Way, Lot #78C, Lawrenceville, GA (Jobsite) - Employees were exposed to fall hazard.  Employees were climbing the frame of the  four buck high tubular welded frame scaffold to reach the working level platforms.  The employees were not utilizing the proper access ladder which was installed on the scaffold, on or about 11/13/14.

Stone Haven Subdivision, 1675 Glen Haven Way, Lot #78C, Lawrenceville, GA (Jobsite) - Employees were exposed to fall hazard.   Employees were working on the 3rd and 4th buck level of the tubular welded frame scaffold without having end rails installed.  Employees were working and walking in close proximity to the edge of the scaffold approximately 18-24 feet above ground level, on or about 11/13/14.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a.  Stone Haven Subdivision, 1675 Glen Haven Way, Lot #78C, Lawrenceville, GA (Jobsite) - Employees were exposed to fall hazard.  The employees were not utilizing a means of fall protection while working and walking approximately 12 feet above ground level on the elevated porch roof removing the work from the elevated area of the two-story residential home, on or about 11/13/14.
