29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    (a). Worksite located at 1306 NW Vivion Road, Kansas City, Missouri- Employees were exposed to overhead hazards from the materials being flown overhead and were not provided any means of head protection.

29 CFR 1926.501(b)(3): Each employee in a hoist area was not protected from falling 6 feet (1.8m) or more to lower levels by guardrail systems or personal fall arrest systems:    (a). Worksite located at 1301 NW Vivion Road, Kansas City, Missouri- Employees were exposed to fall hazards while working near the edge of the building in an attempt to direct and land the suspended load on to the roof structure. Employees were not utilizing any means of fall protection.
