29 CFR 1926.451(e)(8): Direct access to or from another surface was used when the scaffold was more than 14 inches (36 cm) horizontally and/or 24 inches (61 cm) vertically from the other surface:    Location: 60 Pearl Street, Braintree MA    Employees were exposed to falls of approximately 16 feet to the flat roof below where employees were stepping out onto a ladder jack scaffold from a window sill where the horizontal gap was approximately 18 inches.

29 CFR 1926.451(f)(7): Scaffolds shall be erected, moved, dismantled, or altered only under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration. Such activities shall be performed only by experienced and trained employees selected for such work by the competent person.    Location: 60 Pearl Street Braintree, MA     Employees were exposed to fall hazards of 16 feet from un-safe access and lack of fall protection where a ladder jack scaffold had been erected without the supervision of a competent person.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:    Location: 60 Pearl Street, Braintree MA    Employees were exposed to a fall hazard of approximately 16 feet to the flat roof below, while working from a ladder jack scaffold that was not equipped with guardrail systems, end rail systems, nor were personal fall arrest systems used.
