29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a)Guarantee Exteriors, Inc. @ 1000 Massachusetts Ave., Berthoud, CO 80513: On and before November 14th, 2014, the controlling contractor did not ensure that ladders were being used safely onsite. Subcontractors were using portable ladders when side rails were not extended at least 3 feet above the landing surface. Subcontractors were accessing the roof of a house that was approximately 10 feet at the landing surface. This condition exposed subcontractors to a fall hazard of approximately 10 feet.

29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:   (a)Guarantee Exteriors @ 1000 Massachusetts Ave., Berthoud, CO 80513: On and before November 11th, 2014, the controlling employer did not ensure the back D-ring or attachment point of the body harness was on the wearer's back near shoulder level. A subcontractor employee was observed with his rope attached to the front of his harness, not to the D-ring located on the back of his harness.  The employee had the D-ring of the harness located on his lower back, increasing the free fall distance. The employee was exposed to a 10 foot fall hazard.
