The ladder used to access the roof did not extend three (3) feet above the upper landing surface.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards. An employee was exposed to a fall hazard while accessing a residential roof from a portable ladder. The ladder used to access the roof did not extend three (3) feet above the upper landing surface. The employer did not provide training for each employee that would have enabled them to recognize the hazards and provided them with the procedures to follow to minimize these hazards.
