29 CFR 1926.451(b)(4): Each end of a platform, which was not cleated or otherwise restrained by hooks or equivalent means, did not extend over the centerline of its support at least 6 inches:  a. At the site, employees were exposed to fall hazards up to 19 feet high, in that, scaffold platforms extended one to four inches at their end support.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.  a. South east corner of the building, an employee cleaning up debris was exposed to a fall hazard of 19 feet to the ground below, in that, guardrails were not installed on a three frames high tubular welded scaffold tower.
