29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:  On or about November 17, 2014, at the well site called Mojo #1, the employer failed to ensure the employee assigned the task of cleaning the tank was wearing fall protective equipment. Employee was observed walking between tanks and sitting on top of a tank with no fall protection. Employee was exposed to fall hazard of nine feet to the ground below.
