29 CFR 1926.405(a)(2)(ii)(C): Receptacles shall be of the grounding type. Unless installed in a complete metallic raceway, each branch circuit shall contain a separate equipment grounding conductor, and all receptacles were not electrically connected to the grounding conductor:  On or about 11/17/2014, at the above addressed worksite, employees were exposed to a shock hazard due to receptacles that were not connected to the grounding wires.

29 CFR 1926.405(f): Insulated conductors were not distinguishable by appropriate color or other means as being grounded conductors, ungrounded conductors, or equipment grounding conductors:  On or about 11/17/2014, at the above addressed work site, flexible extension cord wiring was not connected to the proper color wire making the polarity indistinguishable exposing employees to a shock hazard.
