29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:    (a)  On or about November 6, 2014, the employer did not protect employees from flying debris on the 8" variable speed bench grinder on the east side of the mechanics shop that did not have the tongue properly adjusted over the abrasive wheel.       Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
