29 CFR 1910.138(a): The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasion; punctures; chemical burns; thermal burns; and harmful temperature extremes: (a) On or about November 3-4, 2014, in the clinic, for a veterinary and a technician administering L-Asparaginase intramuscularly while wearing Mediguard latex exam gloves that had not been tested by the manufacturer for protection against breakthrough with chemotherapy drugs. (b) On or about November 3-5, 2014, in the clinic, for technicians changing the litter of the cat that was administered L-Asparaginase while wearing Mediguard latex exam gloves that had not been tested by the manufacturer for protection against breakthrough with chemotherapy drugs.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: (a) On or about November 3-5, 2014, in the clinic, for a vet and technicians that administered chemotherapy drugs to a cat and cleaned his litter box while he stayed in the clinic for two nights. The employer didn't provide information on the hazards of these drugs prior to their use in the clinic. (b) On or about November 19, 2014, in the clinic, for technicians using products such as isopropyl alcohol, Nolvasan Solution containing Chlorhexidine diacetyl, and Roccal-D-Plus containing Didecyldimethylammonium chloride that had not received training on the new Global Harmonizing System of labeling and the Safety Data Sheets required to be developed by manufacturers under this system.
