29 CFR 1910.22(d)(1): On every building or other structure, or part thereof, used for mercantile, business, industrial or storage purposes, the loads approved by the building official were not marked on plates of approved design and securely affixed in a conspicuous place in each space to which they relate.  (a) Main Building:  Employees were exposed to overhead hazards as the area above the Time Clock in the Parts Area was being used for storage and there was no posted load rating.  (b) Main Building:  Employees were exposed to overhead hazards as the area above the Acetylene tank storage area was being used for storage and there was no posted load rating.  (c) Main Building:  Employees were exposed to overhead hazards as the area above the Oxygen tank storage area was being used for storage and there was no posted load rating.  (d) East Building:  Employees were exposed to overhead hazards as the area above the restrooms was being used for storage and there was no posted load rating.

29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy.  (a) Throughout facility: Employees were exposed to electrical and other hazards as there were no machine specific lockout tagout procedures for machines that had more than one power source, and were hard wired in; or machines that were hard wired in and did not have the disconnects located on the machine, such as, but not limited to, the Corrugation Machine, and the Uni-Hydro Inc. Ironworker,  were not descriptive of the type of machine, the location to lock and tag out the piece of equipment and steps to reenergize machine following maintenance completion.

(b) Head Shop or East Building:  Employees were exposed to the Clark Forklift not being removed from service until restored to a safe operating condition as it did not have a nonslip surface on the break pedal.

29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees, in that the employer did not ensure the safety of the equipment pursuant to the considerations in sections (i) through (viii) of 29 CR 1910.303(b)(1):  (a)  East Building:  Employees were exposed to electrical hazards as the electrical cord for the foot pedal for the Summit Shear had a damaged electrical cord.
