29 CFR 1910.23(c)(1): The employer did not ensure that every open-sided floor or platform 4 feet or more above adjacent floor or ground was guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides.    This violation was established on or about November 12, 2014, where at least five (5) employees working on the mezzanine level were exposed to falls of approximately 8 feet to the concrete below.

29 CFR 1910.132(a): The employer did not ensure that protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were provided, used, and maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.    This violation was established on or about November 12, 2014, where at least five (5) employees who engage in roofing activities are not protected from falling approximately 16 feet to the concrete below.

29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wears a protective helmet when working in areas where there is a potential for injury to the head from falling objects.  This violation was established on or about November 12, 2014, where employees working on the ground floor with roof work above as high as approximately 16 feet were not protected from falling hand tools and materials by the use of hard hats.
