29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-sloped roofs with unprotected sides and edges 6 feet (1.8m) or  more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line systems and guardrail systems, warning line systems and safety net systems, or warning line systems and personal fall arrest systems, or warning line systems and safety monitoring systems:   2221 High School Drive, Brentwood: On 11/20/2014 employees working on a commercial low roof with an approximate fall distance of 22 feet from edge of the roof to ground level were not protected by any conventional fall protection systems.  There was no safety monitoring system in use.  Some employees wore harnesses without having them connected to any anchor points.      The K G framing & Construction, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501 (b)(11), which was contained in OSHA inspection number 710538, citation number 02, item number 1a and was affirmed as a final order on 12/09, 2013 with respect to a workplace located at 1920-1926 McKelvey Road, Maryland Heights, Mo 63043.     The K G Framing & Construction, LLC. was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501 (b)(10), which was contained in OSHA inspection number 315172478, citation number 02, item number 01 and was affirmed as a final order on 12/07/2012, with respect to a workplace located at 120 City Hall Drive, Eureka, MO 63025.     The K G framing & Construction, LLC. was previously cited for a repeat violation of this cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501 (b)(13), which was contained in OSHA inspection number315550368, citation number 01, item number 02 and was affirmed as a final order on 12/07/2012, with respect to a workplace located at 5408 Milentz Ave., St. Louis, MO 63109.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend to at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access nor was the ladder secured at its top to a rigid support and a grasping device, such as a grab rail, provided to assist the employee in mounting and dismounting the ladder:    2221 High School Drive, Brentwood: Side rails of a single ladder used by employees to access an upper surface did not extend, at least, 3 feet above the landing surface.   There was no grab rail in use and the ladder was not secured at the top to prevent movement.    The K & G Framing Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.1053(b)(1), which was contained in OSHA inspection number 710538, citation number 3, item number 3 and was affirmed as a final order on 12/09/2013, with respect to a workplace located at 1910-1926 McKelvey Road, Maryland Heights, MO 63043.   The K & G Framing Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.1053(b)(1), which was contained in OSHA inspection number 314701459, citation number 01, item number 02 and was affirmed as a final order on 01/06/2011, with respect to a workplace located at 4930 St. Louis Avenue, St. Louis, MO 63104.
