29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) On November 5, 2014 employees delivering roofing materials on the roof of a residential construction site, did not utilize the suitable anchorage points that had been installed and were available for use as part of a fall protection system. Employees were exposed to fall hazards ranging from a 12 to 21 feet to the ground below from the variable pitched roof. Note) Certification of corrective action IS required for this item.

29 CFR 1926.503(a)(1): The employer did not provide a training program that would enable each employee to recognize the hazards of falling and train each employee in the procedures to be followed in order to minimize these hazards: a) On November 5, 2014 employees were unfamiliar with a process of continuing having fall protection while transitioning from a ladder positioned at the eave of the roof to a retractable fall protection systems installed at the peak of a roof. The content of the employer's training program did not address the use of a retractable fall protection system in this situation, or how to minimize the employees exposure to a fall hazard during this transition. Note) Certification of corrective action IS required for this item.
