29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a. On or about 20 November, 2014, at 8215 NW Dr., at least one employee performing construction work on a residence without the use of fall protection, was exposed to a 15-foot fall hazard.  b. On or about 20 November, 2014, at 8215 NW Dr., one employee was exposed to an 8-foot fall hazard while using an overhang to access the work area.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards:  On or about 20 November 2014,at 8215 NW Dr., an employee was not being provided with hazard recognition training in regards to step ladder safety, exposing an employee to a 15-foot fall hazard.
