29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a. On or about 11/20/14, at the above mentioned job site (Lot 01/Block 02), an employee on a 3rd floor overhang who was installing plywood with the use of a nail gun and not wearing eye protection was exposed to struck-by hazard.  b. On or about 11/20/14, at the above mentioned job site (Lot 10/Block 03), an employee kneeling on the tie-beam of a 2nd floor removing nails with the use of a hammer and not wearing eye protection was exposed to a struck-by hazard.   PLEASE NOTE: CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a. On or about 11/20/14, at 8401 NW 33rd Street Lot 01/Block 02, employees who were installing plywood on the overhang of a 3rd floor without any means of fall protection were exposed to a fall hazard of approximately 24 feet.  b. On or about 11/20/14, at the above mentioned job site (Lot 10/Block 03), an employee kneeling on a 2nd floor tie-beam who was removing nails from a form board without a means of fall protection was exposed to a fall hazard of approximately 17 feet.    PLEASE NOTE: CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards:   a. On or about 11/20/14, at 8401 NW 33rd Street Lot 01 / Block 02, employees who were not instructed to recognize fall hazards prior to installing and/or removing plywood boards were exposed to a fall hazard.    b. On or about 11/20/14, at 8401 NW 33rd Street Lot 10 / Block 03, employees who were not instructed to recognize fall hazards prior to installing and/or removing plywood boards were exposed to a fall hazard.  PLEASE NOTE: DOCUMENTATION AND CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
