29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  (a) Front side, new residential construction located at 13488 Hipworth Road, Milton, GA 30004: Employees installing cedar shake siding were exposed to eye injuries while operating pneumatic siding nail guns, staplers, and finish nail guns as no protective eyewear was provided while operating these tools at 120 PSI.

29 CFR 1926.304(d): Portable, power-driven circular saw(s) were not equipped with a guard above and below the base plate or shoe:  (a) Cut bench on front side, new residential construction located at 13488 Hipworth Road, Milton, GA 30004: Employee was exposed to amputations while using Makita (Model 5007F) 7-1/4-inch Circular Saws without the lower guard in place. A wood shim was used in both saws to keep the lower blade from retracting when cuts were made.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a) Lower roof, new residential construction located at 13488 Hipworth Road, Milton, GA 30004: Employee was exposed to fall hazards of approximately 14-feet, 1-inch while engaged in cedar shake siding installation on a 3/12 pitched roof with no fall protection system in place.
