29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    (a) South shop: On or about November 18, 2014, and at times prior, employees were exposed to unexpected machine start up while cleaning the #1 Ganesh lathe.    (b) South shop: On or about November 18, 2014, and at times prior, employees were exposed to unexpected machine start up while cleaning the #2 Ganesh lathe.    (c) South shop: On or about November 18, 2014, and at times prior, employees were exposed to unexpected machine start up while cleaning the #3 Ganesh lathe.    (d) South shop: On or about November 18, 2014, and at times prior, employees were exposed to unexpected machine start up while cleaning the #4 Ganesh lathe.                                                                            (e) North shop: On or about November 18, 2014, and at times prior, employees were exposed to unexpected machine start up while cleaning the HL-2 lathe.                                                           (f) North shop: On or about November 18, 2014, and at times prior, employees were exposed to unexpected machine start up while cleaning the ST-30Y lathe.
