29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i:  Engine space Oscar Dyson:  On or about November 18, 2014 and times prior thereto,  a compressed air nozzle was being used that would not reduce to below 30 p.s.i. if dead-ended.  This exposed employees to laceration/air embolism hazards if it contacted their skin.

29 CFR 1910.134(c)(2)(i): The employer did not determine if the voluntary use of a respirator did not in itself create a hazard:  Engine Space Oscar Dyson:  On or about November 18, 2014 and times prior thereto, employees had not given the appendix D of the OSHA standard.  This exposed employees  to potential respiratory hazards from not having a clear understanding of proper respirator use.
