29 CFR 1910.305(a)(2)(i): Temporary electrical power and lighting installations of 600 volts, nominal, or less were used for purposes other than those permitted in subparagraphs (a), (b) and (c) of this paragraph 1910.305(a).  Temporary wiring was used for permanent use:  a- Polymer Dynamix, LLC, South Plainfield, NJ, Manufacturing Area, mezzanine level - Extension cords were used to power machinery that is used on a regular basis in lieu of permanent wiring.  1- Mezzanine level - Ridgid vacuum cleaner  Condition noted on or about 11/21/14.

29 CFR 1910.307(c)(1):  Equipment, wiring methods, and installations of equipment in hazardous (classified) locations were not intrinsically safe or approved for the hazardous (classified) location, or safe for the hazardous (classified) location:    a- Polymer Dynamix, LLC, South Plainfield, NJ - Manufacturing/Plant Areas:      - Micro Air RP-8 Dust Collector     - 25 mm, 50mm and 58mm Extruder Machines     - regular vacuum cleaners  Means were not provided to prevent a fire/explosion hazard, in that, the machinery/equipment and/or wiring used in the Class II combustible locations were not approved for the Class II, Division II locations.  Condition noted on or about 11/21/14. 29 CFR 1910.1200(h)(3)(iv): The details of the hazard communication program developed by the employer, did not include an explanation on the safety data sheet, including the order of information and how employees can obtain and use the appropriate hazard information:  a- Polymer Dynamix, LLC, South Plainfield, NJ - Employee training on the new label elements and the safety data sheet (SDS) format for the revised hazard communication standard (HCS) was not conducted.  Condition noted on or about 11/21/14.
