29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  618 S. Carolina Ave., SE, Third Floor - On or about 11/07/2014, a means of fall protection, including, but not limited to, locking or otherwise restraining a door covering the opening to an elevator shaft, was not provided to a painter who fell 30 feet after opening the door and walking directly into the unguarded shaft.

Carolina Ave., SE, Second Floor - On or about 11/07/2014, construction workers were exposed to a 10 foot fall from a stairway which was not provided with a handrail.

Carolina Ave., SE, First Floor  On or about 11/07/2014, construction workers were exposed to a 10 foot fall from the landing of a stairway which was not provided with standard guradrails.  b)  618 S. Carolina Ave., SE, Second Floor  On or about 11/07/2014, construction workers were exposed to a 10 foot fall from the landing of a stairway which was not provided with standard guradrails.
