OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to fire and carbon monoxide hazards from propane fueled space heaters which were labeled by the manufacturer as unsuitable for indoor use. (a)Sierra's of Colorado, at 1210 W Byers Place, Denver, CO: On and preceding 11/25/14, The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm, in that employees were exposed to fire and carbon monoxide hazards from Dyna-Glo RMC-TT30 propane fueled space heaters which were labeled by the manufacturer as unsuitable for indoor use. The HVAC system for the establishment was disabled and the employer permitted the use of the portable propane fueled space heaters. This condition exposed employees to fire and respiratory hazards associated with exposure to carbon monoxide.

Employees were exposed to toxic chemicals, flying projectiles, and heavy objects during fabrication of countertops. The employer had not certified that a personal protective equipment hazard assessment had been performed. Employees were exposed to the following, but not limited to: 1. Exposure to respirable silica while fabricating granite countertops; 2. Exposure to acetone while assembling stone countertops; 3. Exposure to flying stone debris while polishing, grinding, or cutting stone countertops; and 4. Exposure to heavy objects while transporting stone countertops.

Employees used a Metabo WE 14-125 VS 5" variable speed grinder to fabricate granite countertops. The manufacturer's guards were not is use during stone fabrication operations. This condition exposed employees to a flying projectile hazard.

The outlet on the south wall was not equipped with a faceplate. This outlet was used to power handheld electrical tools used by workers when fabricating stone countertops. This condition exposed employees to electrical shock and burn hazards.
