29 CFR 1904.29(b)(1):  A log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent), was not completed in the detail as required by the regulation:  (a)  On the OSHA 300 Log for calendar year 2014, case number 16:         1.  The employer did not provide the required information on the object or substance that directly injured or made the              person ill; and         2.  The employer did not classify the case whether it was a death, days away from work, job transfer or restriction, or              other recordable case.  (b)  On the OSHA 300 Log for calendar year 2014,  case number 17:        1.  The employer did not provide the required information on the object or substance that directly injured or made the              person ill; and         2.  The employer did not classify the case whether it was a death, days away from work, job transfer or restriction, or              other recordable case.  (c)  On the OSHA 300 Log for calendar year 2014, case number 19:         1.  The employer did not provide the required information on the object or substance that directly injured or made the                person ill; and         2.  The employer did not classify the case whether it was a death, days away from work, job transfer or restriction, or              other recordable case.  (d)  On the OSHA 300 log for calendar year 2014, case number 20:         1.  The employer did not provide the required information on the object or substance that directly injured or made the               person ill; and         2.  The employer did not classify the case whether it was a death, days away from work, job transfer or restriction, or              other recordable case.  (e)  On the OSHA 300 log for calendar year 2014, case number 21:         1.  The employer did not describe parts of the body affected; and         2.  The employer did not provide the required information on the object or substance that directly injured or made the              person ill; and         3.  The employer did not classify the case whether it was a death, days away from work, job transfer or restriction, or              other recordable case.          (f)  On the OSHA 300 log for calendar year 2014, case number 22:         1.  The employer did not provide the required information on the object or substance that directly injured or made the              person ill; and         2.  The employer did not classify the case whether it was a death, days away from work, job transfer or restriction, or              other recordable case.
