29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about 11/25/2014 at the Melekote Medical Building Construction Project, employees were working from the 2nd floor and no fall protection system was in use at the front unguarded opening.

29 CFR 1926.502(b)(1): The top edge height of the top rails, or equivalent guardrail system members, was not 42 inches (1.1 m) plus or minus 3 inches (8 cm) above the walking/working level:  On or about 11/25/2014 at the Melekote Medical Building Construction Project, employees were working from the second floor and the wire rope cable guardrail system was not at 42 inches, plus or minus 3 inches.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  On or about 11/25/2014 at the Melekote Medical Building Construction Project, an employee was working from an aerial lift and using a personal fall protection system that was not rigged to prevent contact with lower levels.
