29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent. Abatement Certification and Documentation is required. On or about 2 December 2014, the employer did not record the following workplace injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2014. 1) An employee working within the warehouse suffered a cut to his left ear resulting in stitches. Date unknown. 2) An employee working at a client's site suffered a shoulder strain. Date unknown.

On 2 December 2014, and at times prior thereto, eight cylinders were being stored adjacent to the main loading dock that did not have the protection caps on them, exposing employees working nearby to the hazard of being struck by a flying projectile.

On 2 December 2014, and at times prior thereto, Acetylene cylinders were being stored adjacent to the main loading dock that were not secured in an upright position while the protection cap was missing, exposing employees working nearby to the hazard of being struck by a flying projectile.
